PIN: This is the same PIN number that you use for online registration - 4 digits example: 2345

Forgot PIN?  Without a PIN number your order will require a written signature for authorization.  After you submit payment, we will provide a way for you to print a summary of your request which you can sign and fax, mail or drop-off to the Registrar’s Office.  You will receive an email confirming the receipt of this form and notifying you that your transcript has been shipped.
